Responsibilities include:

• Implementing and monitoring patient care plans
  
• Monitoring, recording, and communicating patient condition
  
• Serving as a primary coordinator of all disciplines for well-coordinated patient care
  
• Assessing and coordinating patient's discharge planning needs
  
• Providing direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization
